Roc\u00edo Bautista<br>Investigador en Ciencias M\u00e9dicas \u201cD\u201d, Laboratorio de Biolog\u00eda Molecular, INC<\/strong><\/p>\n\n\n\n<p class=\"has-text-align-left\">Un problema de salud muy importante en la actualidad, no s\u00f3lo en M\u00e9xico, sino a nivel mundial es la enfermedad renal cr\u00f3nica (antes insuficiencia renal cr\u00f3nica) producida por la hipertensi\u00f3n arterial y la diabetes, que generan lesi\u00f3n renal en mayor proporci\u00f3n que la ocasionada por padecimientos propios del ri\u00f1\u00f3n como las glomerulonefritis. Para evitar esta enfermedad lo m\u00e1s importante es la prevenci\u00f3n, por lo que hay que reconocerla para impedir la progresi\u00f3n del da\u00f1o renal a etapas terminales, as\u00ed como encontrar la etiolog\u00eda.<\/p>\n\n\n\n<p>Al respecto, es muy importante conocer que la insuficiencia renal se presenta sin muchos s\u00edntomas<br>cl\u00ednicos y la elevaci\u00f3n de la creatinina s\u00e9rica s\u00f3lo ocurre cuando se ha perdido 75% de la masa renal (ambos ri\u00f1ones), por eso es dif\u00edcil de diagnosticar. Lo que significa que la creatinina s\u00e9rica puede estar un poco elevada, 1.5 mg\/dl, pero de aqu\u00ed en adelante su aumento es progresivo. Aunque la elevaci\u00f3n de la creatinina s\u00e9rica nos da un indicio para estudiar la funci\u00f3n renal, una vez que ocurre el ascenso ya es tarde para recuperar la funci\u00f3n perdida y \u00e9sta s\u00f3lo se puede conservar por un tiempo limitado. La depuraci\u00f3n de creatinina es un buen indicador de la funci\u00f3n renal cuando se hace una buena recolecci\u00f3n de orina de 24 horas, pero el m\u00e9dico debe explicarle al paciente c\u00f3mo hacerla, ya que uno de los errores m\u00e1s frecuentes es una recolecci\u00f3n incompleta o en exceso. En ambos casos, los resultados son err\u00f3neos.
